Jaehyuk Choi Delivers Final Grand Rounds Presentation of the Year
On Tuesday, April 3, 2018, Jaehyuk Choi DMD 18 presented the final Grand Rounds case of the year on his externship experience in Port Charlotte, Florida. Dr. Carl McManama moderated the session, which was also attended by Dr. Celeste Kong, Professor and Chair, Department of General Dentistry.
While on externship at the Family Health Centers of Southwest Florida, Mr. Choi encountered a 69-year-old female patient complaining of multiple broken fillings and a missing crown. The patient had good oral hygiene, but upon further examination, an extensive list of problems was found.
The patient had a dislodged crown on #19, overhanging amalgam restoration on #13, buccal cusp fracture on #13, mesiolingual cusp fracture on #18, marginal ridge fracture on #20, caries on #14, partial edentulism on #30 and slight super-eruption of #3. The patient also had financial limitations and had budgeted $3,000 for treatment.
Mr. Choi developed a four-part treatment plan and alternative treatment plan in case of limitations. After a thorough assessment and referrals to an oral surgeon and periodontist, Mr. Choi recommended stabilization treatment including: surgical stent fabrication and implant on lower right, preparation of #20 for monolithic zirconia inlay, preparation of #13, #18, #19, and #29 for monolithic zirconia crowns, and digital impressions for those teeth. The third phase – the treatment and restoration phase – would see the delivery and cementation for # 13, #18, #19, #29, and #20, as well as the delivery and insertion of a screw-retained monolithic zirconia implant crown for #30.
After presenting the plan to the patient and assessing the financial limitations, an alternative treatment plan was decided upon. First, the referral for the #30 implant was not made. Secondly, #20 and #29 were not prepped for inlays. Lastly, Class II DO amalgam restorations would be done for #20 and #29, and no screw-retained implant crown would be used on #30.
